It can be a challenging time for people with disabilities when they transition from being in the hospital to returning home and back into the community.

Having support and assistance during this transition can make a big difference in their ability to successfully adjust to being back at home. 

Making the Transition Smooth

If you or a loved one is transitioning from the hospital to home with a disability, there are several things you can do to make the transition as smooth as possible:

Communicate with your healthcare team

Coordinate with your support coordinator

Check to see what services and equipment (such as a wheelchair or home health aide) are covered under your plan.

Make necessary home modifications

Depending on your needs, you may need to make some adjustments to your home to make it more accessible. This could include installing ramps or handrails or making changes to your bathroom to make it easier to use.

Consider hiring in-home support

If you need help with activities of daily living, such as bathing or dressing, you may want to consider hiring in-home support. This could be a home health aide, nursing assistant, or other professional trained to provide these types of services.

Take care of yourself

It’s important to take care of your physical and mental health during this transition. Make sure you are getting enough rest, eating a healthy diet, and participating in activities you enjoy. We can also help you to connect with a support group or therapist to help you cope with any challenges you may be facing. 

From Prison to Community  

The transition from prison to the community can be challenging for many people.

It is common for individuals who have been incarcerated to face a number of barriers when trying to reintegrate into society, including difficulty finding housing and employment, as well as stigma and discrimination. 

There are many programs and resources available to help individuals successfully transition from prison to the community.  

These include job training and placement programs, housing assistance, mental health, and substance abuse treatment, and support groups.

It is also important for individuals who are returning to the community to have a solid support system in place, such as family, friends, or community organisations.  

Having a network of people who can offer emotional and practical support can be crucial in helping individuals successfully reintegrate into society. 

It can also be difficult to re-establish relationships with family and friends, and to adjust to life outside of prison. 

A customised support team can provide tailored assistance to individuals with disabilities in order to help them live as independently as possible.  

This type of support team may include professionals such as social workers, therapists, and personal care assistants, as well as family members and other caregivers.  

The specific roles and responsibilities of the team will depend on the needs and goals of the individual with a disability.  

The team may help with tasks such as personal care, transportation, communication, accessing community resources, and implementing positive behaviour support plans.  

It is important for our team to work closely with individuals with a disability and to involve them in decision-making to ensure that the support provided meets their needs and preferences.
