Your Independence Matters the Most
Setting your goals is where our NDIS service begins.
Our Team Leaders sit with our clients to review their goals and to breakt them down into monthly goals and weekly goals, and then daily activities.
Our goal is to make progress and to hit targets weekly so that the monthly goals and yearly goals are achieved.
Setting goals in a care plan can effectively ensure that a person’s care is tailored to their specific needs and preferences.
Our 5 Main Points for Setting Goals in a Care Plan
Involve the Person Receiving Care
Including participants in the goal-setting process is critical so the goals are realistic and achievable within reasonable timeframes.Involving them will help ensure that the goals are meaningful to them and that they can achieve them.
Make Objectives Specific
Goals need to be SMART – Specific, Measurable, Attainable, Relevant and Timely. This ensures they are specific and clear to be able to track progress. Goals also need to be realistic given the person’s current situation and resources.
Set Priorities
It is critical to prioritise goals because it may not be possible to work on them simultaneously. The MyCare team considers the individual’s current needs and abilities and the resources at their disposal when setting goals.
Involve the Care Team
Our Team Leaders are involved in goal setting and care plan development. This ensures that the goals are attainable and that the necessary resources and support are in place to assist the individual in achieving their objectives. Because after all, it is a team effort.
Review and Adjust
The care plan is reviewed on a regular basis and updated as needed. This allows goals to be adjusted as the person’s needs and abilities change. If goals aren’t being achieved they are adjusted so that they are obtainable.
